*By submitting this questionnaire you agree to the following terms & conditions*** 1.) RESCHEDULES AND CANCELLATIONS. Please note that no session is reserved until receipt of payment. Cancellation and Rescheduling of Sessions Your session fee is non-refundable. In the event that you cancel your portrait session after it has been scheduled, no refund of your session fee will be made to you. However, as we understand that schedules may change, in the event that you need to reschedule your session and you provide at least forty-eight (48) hours advance notice of same. Your session fee will be applied toward a regular session and you will be responsible for the difference. The session must happen within one month of the originally scheduled mini session. If you prefer an out-of-town location, travel fees will apply and are subject to availability. 2.) SESSION FEE. What is included? The non-refundable fee includes (i) a photography session (ii) professional retouching of images as selected by the photographer; (iii) an online gallery available within two (2) weeks to view and select your images; and (iv) digital download which includes high-resolution for print (with exclusive print release). 3.)TIMELINESS. Sessions are extremely time-sensitive and I depend on the timeliness of my clients to make them go as smoothly as possible. Only one outfit change is recommended for your half session. 4.) MODEL RELEASE: Alicia Lucia Photography reserves the right to use images created under this agreement for advertising, display, competition, marketing materials, web site, publication or other purposes. By signing this agreement, Client warrants that he or she has actual authority to agree to the use of the likeness of all persons included in the portrait in this manner and shall indemnity and defend the photographer in the event of litigation arising out of such use. *
